How to report conditions to the health department:
The easiest way to report a reportable condition to Three Rivers is to use our "Report Form" (see below).Simply the form, and fax it and any accompanying lab report to our office using our secure fax line: 402-727-5399.
Conditions can also be called into the health department at 402-727-5396 ext 19.
Please have the following information ready when a report is called in:
- Patient's name and contact information
- Diagnosis or suspected diagnosis (morbidity report)
- Date of onset of symptoms/illness
- Diagnosis date
- Treatment
- Hospitalization information if appropriate
Nebraska's Reportable Diseases, Poisonings, Organisms & Events
Immediately Reportable There are 28 diseases, poisonings, organisms & events that physicians, hospitals, and laboratories are required to immediately report to the health department.
Report Within Seven Days
There are 48 diseases, poisonings, organisms & events that must be reported to the health department by physicians, hospitals, and laboratories, within seven days of diagnosis.
