The clinic is located in the Three Rivers District Health Department offices, at 61 W 4th St,Fremont

Appointments are available every Monday (9-12 & 1-6) and Thursday (9-12), excluding holidays. Please call to make an appointment: 402-727-5396 ext. 4

Please bring previous immunization records to your appointment.

Parent or legal guardian must accompany minors.

For children's immunizations, an $8.00 donation per shot is requested.

For adult immunizations, prices vary according to individual vaccines.

No child is denied services.

Late arrivals may require a wait for the next available appointment.

There is also a Satellite Office in Wahoo located at 1102 North Chestnut, Wahoo.

Appointments are available the first Wednesday of every month, excluding holidays. We offer appointments throughout the month as needed, when available. Please call to make an appointment: 402-443-4603

We provide immunizations for children and adults according to the Vaccine for Children program which is approved by the Center for Disease Control and the State of Nebraska Immunization Program.
